When mailing your application/materials, please send them to: ADE – Certification P.O. Box 6490 Phoenix, Arizona 85005-6490 Applicants who are submitting an application via mail must submit payment in the form of a check or money order. Cash payments are not accepted.

The Phoenix Certification office accepts personal checks, money orders, cashier’s checks, and credit card payments (Visa and MasterCard only) for in-person applicants at the Certification customer service counter. Credit card payments will only be accepted in person.
