FY23 Single Audit Qualifier Questionnaire

Single Audit Qualifier Questionnaire Description

This questionnaire will collect responses from all operators of the Child and Adult Care Food Program (CACFP), and collect a copy of the single audit report for those who qualified for a single audit for fiscal year 2023 (FY 23). Arizona Department of Education, Health and Nutrition Services (HNS) will monitor the submissions. 

All sponsors that received Federal funds from ADE are required to complete a Single Audit Qualifier Questionnaire to report to ADE if the annual amount of federal funds expended qualify the entity to conduct a single audit. Any entity that expends more than $750,000 in Federal funds in a fiscal year is required to hire an independent Certified Public Accountant (CPA) to conduct a single audit to review both the compliance and financial component of Federal programs. Operators must ensure the completion of the single audit is completed within 12 months after the close of the fiscal year.

Welcome and thank you for your interest in the Child and Adult Care Food Program.  The Child and Adult Care Food Program (CACFP) is a federal program that provides reimbursements for nutritious meals and snacks to eligible children and adults who are enrolled for care at participating child care centers, day care homes, and adult day care centers. CACFP also provides reimbursements for meals served to children and youth participating in afterschool care programs, children residing in emergency shelters, and adults over the age of 60 or living with a disability and enrolled in day care facilities. CACFP contributes to the wellness, healthy growth, and development of young children and adults in the United States.

The first step towards participating in the CACFP program is to complete this CACFP Prospective Entity Form for Program Operators. All forms needed can be found on the Required Entity Organization Application Forms webpage - Forms for Prospective Child and Adult Care Food Program Operators accordion.
