Duplicate certificates may not be printed through the AzEDCert portal. This service must be requested via mail or at the customer service counter. Please print the Request for a Duplicate Copy of Certificate and mail it to the address on the form along with the $20 fee.
To apply for an IVP Fingerprint Clearance Card: Please visit the Arizona Department of Public Safety (DPS) website. Select "Fingerprint Clearance Card Application, Requests, and Status Checks" for information on how to apply for your IVP fingerprint clearance card
Name changes may not be updated through the AzEDCert portal. Name changes must be requested via mail or at the customer service counter. Please print the Application to Change My Name and mail it to the address on the form with the appropriate documentation with the $20 fee.
