AZELLA Additional Accommodations Request 2025-2026

This is a request for additional accommodations for one (1) student who is eligible for the administration of the AZELLA and has a current IEP or 504 Plan. The additional accommodations being requested SHOULD NOT include those listed under the Universal Test Administration Conditions or under the "Approved AZELLA Assessment Accommodations." See the AZELLA Accommodations Manual. Requests should not be submitted for students eligible to take Alt ELPA.

One request submission will cover both AZELLA test administrations (Placement and Spring Reassessment) for the student during the current school year. This request is only valid for the current school year. If the student’s IEP or 504 Plan is updated for instruction and state assessments during the school year and after this initial submission, you will need to submit a new request with the updated IEP or 504 Plan.

There are no provisions, in either state or federal law, that would allow the exemption of AZELLA testing for eligible students who are enrolled in the Special Education program and require an AZELLA test. An IEP Team, which should include a staff member with a background on second language acquisition, may NOT exempt a student from participating in AZELLA testing.

Not all accommodations are appropriate for AZELLA testing. Approvable accommodations for AZELLA are those that do not alter the construct that the assessment is intended to measure.

The additional accommodations requested in this form must be included in the accommodations for instruction, district assessments, and state assessments section of the student's IEP or 504 Plan. Accommodations specific for AZELLA should also be included in the student's IEP or 504 Plan. This request must be accompanied by supporting documentation pages extracted from the student's IEP or 504 Plan. 

The submission of this request form does not constitute an approval for the additional accommodations being requested. ADE will review the request and then ADE will provide a response to the AZELLA District Test Coordinator that includes approval, denial, or a request for additional information for each accommodation being requested. The entire review process may take up to 4 calendar weeks from the date of submission provided that the required documentation is submitted in the original request or additional information is submitted in a timely manner upon ADE's request. The student may NOT begin AZELLA testing until ADE's response indicates to proceed with the administration of the AZELLA test. 

Alt ELPA: When the student's IEP Team has determined the student is eligible for Alternate Assessments, then the student must be administered the Alt ELPA in lieu of the AZELLA. For students who are eligible for the Alt ELPA, this AZELLA Additional Accommodations Request form is not necessary. Your Alternate Assessment DTC receives communications and guidance from ADE's Alternate Assessment Team regarding training and timelines for the administration of the Alt ELPA tests.

  • Alt ELPA Screener (in lieu of the AZELLA Placement Test)
    • The test administration compliance time frame for the Alt ELPA Screener matches the AZELLA Placement Test.
      • When the student is eligible for Alternate Assessments (Alt ELPA) and the student has an AZELLA or Alt ELPA record with an Overall Proficiency Level of less than Proficient from the prior school year (2024-2025), the student should NOT be administered the Alt ELPA Screener. This student must be administered the Alt ELPA Summative during the Spring ELP testing window.
  • Alt ELPA Summative (in lieu of the Spring AZELLA Reassessment)

 

If this request is ONLY for the single accommodation to administer the AZELLA test in more than 5 instructional days, please DO NOT complete this request. Please identify the district and school where the student was recently enrolled prior to enrolling in your school. Do not use acronyms. Please use the entire district and school names and their entity numbers, if known.
Will the student be able to access all AZELLA domains (Listening, Reading, Writing, and Speaking) WITH allowable and approved accommodations?
CHECK NO MORE THAN 3. Identify the AZELLA Domain(s) in which the Student's Disability Precludes Participation (CANNOT ACCESS due to disability - e.g., completely deaf and/or completely blind when the student does not use any accommodations for the disability). Whenever administering any AZELLA test, the entire test must be administered. There are no exemptions to being administered a full AZELLA test. However, if, even with accommodations, a student is UNABLE to ACCESS one or more of the AZELLA domains because the student has a disability that precludes his or her participation in that domain, you must identify the domain(s) that the student’s IEP or 504 Plan have indicated that student is not able to access. For example, a student who is completely deaf and/or completely blind may not be able to "access" specific AZELLA domains. However, a student with a speech disability CAN "ACCESS" the Speaking Test and must be provided an earnest attempt with the AZELLA Speaking Test unit; therefore, the student is not eligible for the Speaking test exception. A student may NOT be exempt from ALL the AZELLA domains.
I attest that the student for whom I am submitting this request for additional accommodations experiences blindness or deafness that is permanent in nature and precludes access to one or more of the AZELLA domains. I also attest that the need for additional accommodations submitted to and approved by the Arizona Department of Education has not changed since the initial request.

Does the student use assistive technologies?
Enter the official names, type (Switch, AAC, Mobility, etc.), and the primary task that the device supports for the devices that are included in the student's IEP or 504 Plan, and that the student routinely uses during instruction and assessment. What is the level of independence with device(s)? Describe how the student controls each device (e.g., eye gaze, voice input, pressing a button, etc.) Do any of the devices have Internet access? If so, can the Internet be disabled on those devices?

Addressing EL Needs

Please provide documentation and information pertinent to the student's EL needs and how they are being addressed in daily instruction. You may include additional comments and information that is of value for the review of this request.

File Uploads

Not all accommodations are appropriate for AZELLA testing. Accommodations may not alter the construct of the assessment. Please provide documentation pertinent to the student’s EL needs and how they are being addressed in instruction.

You must submit page(s) of the ACTUAL sections (not the entire IEP/504 Plan) of the student’s official IEP or 504 plan that indicate the needed accommodations are used routinely during instruction and assessments throughout the year. "AZELLA" State Test (Grades K-12) must be identified in the State Assessment section of the IEP/504 Plan. These pages must have the student’s SSID number, demographics, and the date of the document on each page submitted. Do not submit the entire IEP or 504 Plan.

The following 2 sections must be included with this submission:

  • PLAAFP (Present Levels of Academic Achievement and Functional Performance)
  • Instructional and Assessment Accommodations

AZELLA Test Security and Ethics Incident Report - 2025-2026

This report is used to report test security and ethics violations of the AZELLA before, during, and after test administration. All incidents must be thoroughly investigated by the district and school prior to submitting this form. When the test security and ethics incidents involve the AZELLA test, this secure report needs to be submitted to the Arizona Department of Education's Assessments AZELLA Team within 24-48 hours of the incident. ADE will respond within 3 full business days of the submission. If this incident involves students testing, the students may not resume testing until ADE provides the DTC with a response to the incident.

Large-scale assessments like the AZELLA are developed, administered, and scored to meet comprehensive standards of validity, reliability, and fairness for a wide range of learners. In assessment, reliability is the characteristic of an assessment that allows its results to be repeatable. A reliable assessment will produce stable and consistent results. We can ensure these standards are maintained by closely adhering to security policies throughout test administration and conducting the assessment in a fair and ethical manner.

The Arizona Department of Education, or ADE, is committed to taking every step required to assure the security and confidentiality of all state test materials, including the AZELLA.

All AZELLA staff whether they are inventorying, distributing, or administering the tests, or just have incidental access to AZELLA materials must abide by these security standards. Failure to do so could negatively impact the validity of the AZELLA, and the results of the test. It could also lead to allegations of test impropriety.

ACT Aspire

ACT Aspire is the statewide high school achievement test for Arizona students in the Grade 9 cohort. ACT Aspire will be administered to students in Cohort 2029 in the 2025-2026 school year.

While ACT Aspire has been discontinued at a national level, Arizona will continue to administer the ACT Aspire Early High School assessments to students in the Grade 9 cohort for the life of our contract. As of November 1, 2023, support for ACT Aspire has now shifted from ACT to Pearson.

The AZELLA Placement Test window begins on the the district's or charter's first instructional day of the school year. However, ADE has approved a 2-calendar week jump start prior to the first day of school to begin Placement testing.
