This AZELLA Test Security and Ethics Incident Report is used to report AZELLA test security and ethics violations for Grades K-12 AZELLA Placement Tests, the annual Spring Reassessment Test, and the 2021 Stand-Alone Field Test. All AZELLA test security and ethics violations must be reported to the Arizona Department of Education's AZELLA Team within 24 hours of the incident. This report is a secure online form.

Interpreting test items or passages for students
Rewording or clarifying questions or using inflections or gestures to help students choose the correct answer
Allowing students to use unauthorized resources to find answers, including having materials on walls or additional resources such as books and worksheets that provide answers to specific test questions or assists the student with responses
Reading or signing test content to students except when scripts of the Stages 1 and 2 Test Administration Directions explicitly indicate to do so
Reviewing, discussing, and assisting a student with his or her answers, instructing or suggesting to a student to rethink his or her answers
Explicitly or implicitly encouraging students to engage in dishonest testing behavior
Allowing school or district staff to administer or assist with proctoring of the test their child is taking
Allowing parent volunteers to assist with the proctoring of a test their child is taking
Using students to supervise other students taking a test
Erasing or changing student responses or any marks on a scorable test booklet or answer document, and student responses associated with an online test
Developing scoring keys or preparing answer documents
Using testing materials for instruction before, during, or after test administration
Administering the AZELLA without signing the AZELLA Test Security Agreement and/or completing required training modules first
Not following all instructions in the Test Administration Directions including reading the directions to students exactly as scripted
