Secondary Transition
Mission
To ensure that all public education agencies in Arizona are able to receive quality technical assistance in the planning, development, organization, implementation and evaluation of special education secondary transition services.
Required Components of Transition
Beginning no later than the first IEP to be in effect when the child is 16, the following components must be included in the IEP and must be updated annually thereafter:
- Student Invite - The child must be invited to the IEP meeting where transition services are likely to be discussed
- Measurable Postsecondary Goal(s), updated annually – Goal statements related to further education/training, employment, and where appropriate, independent living, and are based upon age-appropriate transition assessments
- Age-appropriate transition assessments – Assessments that provide information on the child’s strengths, interests and preferences are used to write achievable, measurable postsecondary goals
- Coordinated set of activities – Transition-related services or activities that reasonably enables the child to achieve their stated measurable postsecondary goal(s)
- Courses of study – Courses that will reasonably enable the child to achieve their stated measurable postsecondary goal(s)
- Outside agency invite with prior consent - Invitation of an outside agency to the IEP meeting if the agency is likely to pay for transition services, with the prior consent of the parent or child who has reached the age of majority
- Annual IEP goal(s) – Annual IEP goals that relate to the measurable postsecondary goal(s)
- Transfer of Rights – A statement that informs the child that rights will transfer to the child at the age of majority
- Summary of Performance – For a child whose eligibility terminates due to graduation from high school with a regular diploma, or due to exceeding the age eligibility for FAPE under State law, a local education agency must provide the child with a summary of the child’s academic and functional performance, which shall include recommendations on how to assist the child in meeting the child’s postsecondary goals.

Organization that facilitates interaction and shared work across professional and family organizations around common interests.
Mountain Plains Regional Resource Center (MPRRC)
OSEP funded project that helps build the capacity of State Education Agencies and Lead Agencies in improving programs and services for infants, toddlers, children and youth with disabilities
National Center on Educational Outcomes (NCEO)
Organization that provides national leadership in the participation of students with disabilities in national and state assessments, standards-setting efforts and graduation requirements
National Dropout Prevention Center for Students with Disabilities (NDPC-SC)
A Technical Assistance and Dissemination Network specifically established to assist in building states’ capacity to increase school completion rates for students with disabilities through knowledge synthesis, technical assistance and dissemination of interventions and practices that work.
National Post School Outcomes Center (NPSO)
The mission of this center is to help state education agencies establish practical and rigorous data collection systems that will measure and profile the post-school experience of youth with disabilities
National Secondary Transition and Technical Assistance Center (NSTTAC)
OSEP-funded organization that assists states to build capacity to support and improved transition planning, services and outcomes for youth with disabilities. Focuses on assisting with collecting data on Indicator 13 and using the data to improved transition services

Armed Services Vocational Aptitude Battery (ASVAB)
This program is designed to help students learn more about themselves and the world of work, identify and explore potentially satisfying occupations and develop an effective strategy to realize their goals.
Arizona Career Information System (AzCIS)
Provides educational, career and occupational information to help all students in making better-informed career and school choices. Free to Arizona residents. To request a username and password, call 602-542-5352.
Career InfoNet – Career Tools
U.S. Department of Labor website that provides information on careers (i.e., skills necessary, videos, etc.). Have your skills matched to occupations, or check skills required for an occupation.
Colorado Department of Education – Transition Toolkit
A Toolkit developed to provide information and tools necessary in creating a comprehensive and individualized transition process. Modules in the Toolkit include Transition Planning Basics, various assessments, IEP development using the collected information, tools for students, teachers/transition teams and parents.
Iowa Transition Assessment
Provides resources and tools to assist in identifying the interests, preferences, strengths and needs of students with IEPs so students will be better prepared for employment, further education and independent living.
NCWD/Youth – Career Planning Begins with Assessment: A Guide for Professionals Serving Youth with Educational and Career Development Challenges
A career planning guide for multiple audiences including youth service practitioners and was deliberately developed with numerous quick reference charts, tables and sample forms. Including a chapter that contains a directory of assessments with a description of each, the website where it can be found, the cost (if any) and publisher information.

A document that provides an overview of transition assessments, why they are necessary, how to conduct them and sample instruments.
Self-Determination Assessments - Zarrow Center for Learning Enrichment
This Center facilitates successful secondary and postsecondary educational, vocational and personal outcomes for students and adults with disabilities. This assessment scale produces a profile of the student’s level of self-determination, identifies areas of strength and areas needing improvement and identifies specific educational goals that can be incorporated into the student’s IEP.
South Dakota’s Quickbook of Transition Assessments
A Transition Planning Guide that contains checklists, Interview Guides, suggested planning activities and assessments.
Test Prep Review
Resource for FREE online practice tests, including ASVAB, SAT, PSAT, ACT, etc.
Transition Assessment Online Training Module
Transition Coalition provides FREE research-based online training for professionals and others involved in transition planning.
Virginia Department of Education Assessment Matrix
A matrix that lists a sampling of different assessments that can be used when planning transition services. The matrix includes the name of the assessment, which area of transition planning it pertains to (i.e., employment, education/training, independent living/community participation), what age group or grade level the assessment is appropriate for, whether it is a web-based assessment and the publisher’s contact information.

The mission of DCDT is to promote national and international efforts to improve the quality of and access to career/vocational and transition services, increase the participation of education in career development and transition goals and to influence policies affecting career development and transition services for persons with disabilities.
SharedWork
A website designed to support and facilitate the shared work that occurs among individuals, organizations and agencies at the local, district, state and national levels. There is a page dedicated to a Community of Practice on Transition where the Arizona page can be found. You must register in order to enter the site, registration is free.
Transition Coalition (University of Kansas)
Provides online information, support and professional development on topics related to transition from school to adult life for youth with disabilities.
Transition of Students with Disabilities To Postsecondary Education: A Guide for High School Educators (an Office for Civil Rights publication)
A booklet published by OCR to inform educators about the rights and responsibilities of students with disabilities and to facilitate their transition from high school to postsecondary environments.
