- Career InfoNet – Career Tools
U.S. Department of Labor website that provides information on careers (i.e., skills necessary, videos, etc.). Skills can be matched to occupations, or skills required for an occupation can be researched. - Colorado Department of Education – Transition Toolkit
Developed to provide information and tools necessary in creating a comprehensive and individualized transition process. Toolkit Modules include Transition Planning Basics, various assessments, IEP development using collected information, tools for students, teachers, transition teams and parents. - Iowa Transition Assessment
Provides resources and tools to assist in identifying the interests, preferences, strengths and needs of students with IEPs so students will be better prepared for employment, further education, and independent living. - NCWD/Youth – Career Planning Begins with Assessment: A Guide for Professionals Serving Youth with Educational and Career Development Challenges
A career planning guide for multiple audiences including youth service practitioners. Deliberately developed with numerous quick reference charts, tables, and sample forms with a description of each, the website where it can be found, the cost (if any) and publisher information. Includes a chapter that contains a directory of assessments.

Provides an overview of transition assessments, why they are necessary, how to conduct them, and sample instruments. - Self-Determination Assessments – Zarrow Center for Learning Enrichment
This Center facilitates successful secondary and postsecondary educational, vocational, and personal outcomes for students and adults with disabilities. The assessment scale produces a profile of the student’s level of self-determination, identifies areas of strength, areas needing improvement, and specific educational goals that can be incorporated into the student’s IEP. - South Dakota’s Quickbook of Transition Assessments

Transition Coalition provides FREE research-based online training for professionals and others involved in transition planning. - Virginia Department of Education Assessment Matrix
A matrix that lists a sampling of different assessments for use when planning transition services. Includes the name of the assessment, which area of transition planning it pertains to (i.e., employment, education/training, independent living/community participation), age group or grade level the assessment is appropriate for, whether it is a web-based assessment, and the publisher’s contact information.
