Mission
To ensure that all public education agencies (PEAs) in Arizona are able to receive quality technical assistance in the planning, development, organization, implementation and evaluation of special education secondary transition services.

Required Components of Transition

Beginning not later than the first IEP to be in effect when the child turns 16, or younger if determined appropriate by the IEP team, the following components must be included in the IEP and must be updated annually thereafter:

  • Measurable Postsecondary Goal(s) – Goal statements related to further education/training, employment, and where appropriate, independent living
  • Updated Annually
  • Based on Age-Appropriate Transition Assessments – Assessments that provide information on the child’s strengths, interests and preferences are used to write achievable, measurable postsecondary goals
  • Transition Services = Coordinated Set of Activities – Transition-related services or activities that reasonably enable the child to achieve his/her stated measurable postsecondary goal(s)
  • Transition Services = Courses of Study – Courses that will reasonably enable the child to achieve his/her stated measurable postsecondary goal(s)
  • Annual IEP goal(s) – An annual IEP goal that relates to the measureable postsecondary goal
  • Student Invitation – The child must be invited to the IEP meeting where transition services are likely to be discussed
  • Outside Agency Invitation (with prior consent) – Prior to inviting an outside agency that is likely to pay for or provide transition services, consent from the parent, or the child who has reached the age of majority, must be obtained
  • Transfer of Rights – A statement informing the child that rights will transfer to the child at the age of majority
  • Summary of Performance – For a child whose eligibility terminates due to graduation from high school with a regular diploma, or due to exceeding the age eligibility for FAPE under State law, a PEA must provide him/her with a summary of academic and functional performance, which shall include recommendations on how to assist in meeting his/her postsecondary goals

Technical Assistance Centers
  • National Secondary Transition and Technical Assistance Center (NSTTAC)
    OSEP-funded organization that assists states to build capacity to support and improve transition planning, services, and outcomes for youth with disabilities. Focuses on assisting with collecting data on Indicator 13 and using the data to improve transition services.
  • National Post School Outcomes Center (NPSO)
    OSEP-funded organization that helps states establish practical and rigorous data collection systems that will measure and profile the post-school experience of youth with disabilities.
  • National Dropout Prevention Center for Students with Disabilities (NDPC-SD)
    OSEP-funded organization that helps states build capacity to increase school completion rates for students with disabilities through knowledge synthesis, technical assistance, and dissemination of interventions and practices that work.
  • IDEA Partnership
    Organization that facilitates interaction and shared work across professional and family organizations around common interests.
  • Mountain Plains Regional Resource Center (MPRRC)
    OSEP-funded project that helps build the capacity of state education agencies (SEAs) in improving programs and services for infants, toddlers, children, and youth with disabilities.
  • National Center on Educational Outcomes (NCEO)
    Organization that provides national leadership in the participation of students with disabilities in national and state assessments, standards-setting efforts, and graduation requirements.

    • NSTTAC – Age Appropriate Transition Assessment Guide
      Provides an overview of transition assessments, why they are necessary, how to conduct them and sample instruments.
    • Self-Determination Assessments – Zarrow Center for Learning Enrichment
      This Center facilitates successful secondary and postsecondary educational, vocational and personal outcomes for students and adults with disabilities. The assessment scale produces a profile of the student’s level of self-determination, identifies areas of strength, areas needing improvement, and identifies specific educational goals that can be incorporated into the student’s IEP.
    • South Dakota’s Quickbook of Transition Assessments
      Transition planning guide that contains checklists, interview guides, suggested planning activities and assessments.
    • Test Prep Review
      Resource for FREE online practice tests, including ASVAB, SAT, PSAT, ACT, etc.
    • Transition Assessment Online Training Module
      Transition Coalition provides FREE research-based online training for professionals and others involved in transition planning.
    • Virginia Department of Education Assessment Matrix
      A matrix that lists a sampling of different assessments for use when planning transition services. Includes the name of the assessment, which area of transition planning it pertains to (i.e., employment, education/training, independent living/community participation), age group or grade level the assessment is appropriate for, whether it is a web-based assessment, and the publisher’s contact information.

    • Division on Career Development & Transition (DCDT)
      The mission of DCDT is to promote national and international efforts to improve the quality of and access to career/vocational and transition services, increase the participation of education in career development and transition goals and to influence policies affecting career development and transition services for persons with disabilities.
    • SharedWork
      A website designed to support and facilitate the shared work that occurs among individuals, organizations and agencies at the local, district, state and national levels. There is a page dedicated to a Community of Practice on Transition where Arizona information can be found. You must register in order to enter the site; registration is free.
    • Transition Coalition (University of Kansas)
      Provides online information, support and professional development on topics related to transition from school to adult life for youth with disabilities.
    • Transition of Students with Disabilities To Postsecondary Education: A Guide for High School Educators (an Office for Civil Rights publication)
      A booklet published by OCR to inform educators about the rights and responsibilities of students with disabilities and to facilitate their transition from high school to postsecondary environments.
