In connection with A.R.S. § 15-185 (B) (1) (b), the Arizona Department of Education (ADE) is collecting the following information from all charter holders to determine the correct application of the small school weights for each charter. Therefore, each charter holder must complete and submit this Statement of Assurance regarding its organizational structure before [June 15, 2015]. Charter holder has the definition given in A.R.S. § §15-101(3): “a person that enters into a charter with the state board for charter schools. For the purposes of this paragraph, ‘person’ means an individual, partnership, corporation, association or public or private organization of any kind.”
A charter holder or management company may elect to submit a single statement of assurance for all of its charters. Fields will be provided to enter information for all affiliated charters.
This form must be completed by a person who is authorized to do so.

Schools and school districts are required, pursuant to Arizona Revised Statutes section 15-142, to release pupil directory information by October 31 each year. The statute also requires ADE to develop a model form to assist districts and charters in complying with this requirement.
