Applications for Initial Certificates, Endorsements, or Approved Areas

Application for Certification – Used to apply for most certificates (Substitute, Initial Teaching, Administrative, Professional Non-Teaching) and to add approved areas and endorsements to existing certificates.

Application for Reciprocal Teaching and Reciprocal Administrative Certificates – Used by teachers and administrators who are certified out-of-state.

Application for Teaching Intern Certificate -Used to apply for a Teaching Intern certificate. Note: The Teaching Intern certificate allows the holder to enter into a teaching contract while completing a Board authorized alternative path to certification or a Board authorized teacher preparation program.

Application for Interim Administrative Certificate

Application for Provisional Foreign Teacher Certificate – Used by applicants who are participating in a foreign teacher exchange program.


Applications for Extension

Application to Extend a Reciprocal or Provisional Certificate – Used to extend a Three-Year Provisional Elementary, Provisional Secondary, Provisional Special Education, Provisional Arts Education, or Provisional Early Childhood Certificate because you do not have the required full-time teaching experience to qualify for a Standard teaching certificate. This application may also be used to extend  One-Year Reciprocal or a One-Year Provisional certificates that are eligible for extension due to State Board rule changes.

Application to Extend a Provisional Career and Technical Education Certificate – Used to extend a Three-Year Provisional Career and Technical Education (CTE) certificate.

Applications for Conversion to a Standard Certificate

Application to Convert a Provisional Arts Education, Early Childhood, Elementary, Secondary, or Special Education Certificate – Used to convert Provisional Arts Education, Early Childhood, Elementary, Secondary, or Special Education Certificate to a Standard teaching certificate.

Application to Convert a Reciprocal Provisional Certificate – Used to convert a Reciprocal Provisional Arts Education, Early Childhood, Elementary, Secondary, or Special Education Certificate to a Standard teaching certificate.

Application to Convert a Provisional Career and Technical Education Certificate – Used to convert a Provisional Career and Technical Education (CTE) certificate to a Standard CTE certificate.

Application to Convert a Provisional Adult Education Certificate – Used to convert a Provisional Adult Education to a Standard Adult Education certificate.


Applications for Renewal

Application for Renewal of Certificate – Used by applicants who hold a Standard Teaching, Administrative (Principal, Superintendent, or Supervisor), Guidance Counselor, School Psychologist, Athletic Coaching, or Native American Language certificate.

Change of Address Request

Application to Remove Deficiencies – Use this form to remove the Arizona constitution, US constitution, and/or Fingerprint Clearance card deficiencies.

Application for Name Change/Duplicate Copy of Certificate

Verification of PreK-12 Teaching Experience – Used to verify full-time teaching experience.

Verification of Career and Technical Education (CTE) Occupational Experience – Used to verify work experience in the appropriate CTE occupation area to qualify for initial issuance of a Provisional CTE certificate.

CTE Teaching Evaluation Verification – Used by applicants who have held a Provisional Career and Technical Education (CTE) certificate for at least two years and are applying for conversion of a Provisional CTE certificate to a Standard CTE certificate.

Explanation of Incident Form – Used by applicants who answer “Yes” to a Criminal History or Background Questions.

Foreign Credential Evaluation Agencies List of foreign credentialing agencies for applicants who are using coursework or degrees from a foreign college or university to meet certification requirements.
