Students with Chronic Health Conditions: A Proactive Approach to Transition

The ADE School Safety and Social Wellness and ADE/ESS Secondary Transition teams, in partnership with the Mayo Clinic, are excited to announce a virtual professional learning opportunity on Wednesday, April 28, 2021 from 2:30–3:30 p.m. for school nurses, school social workers, school psychologists, and school counselors. Although the focus will be specific to students in grades 6–12, anyone is welcome to attend.
The purpose of this presentation is to raise awareness regarding students with chronic health conditions and how the education system can empower families to advocate for their children during their school years. Many children are diagnosed with a chronic medical condition prior to graduating from high school. This diagnosis may impact their childhood in a variety of ways. Often, families are not aware of additional resources available to their child and do not consider consulting with the school for such guidance. This presentation will highlight educational needs as well as identify ways to better prepare students for their transition into adulthood when they have a chronic health condition and/or disability.
