The 4 menu records that schools operating NSLP are required to maintain:
1) Production Records
Separate daily production records for breakfast and lunch are required
Production Record Template (no menu contributions)
Production Record Template (with menu contributions)
Production Record for Multiple Grades
2) Nutrition Facts Labels
Required for all food items served as part of food service
3) Standardized Recipes
Required to for all menu items that are made of more than 1 item.
(Ex: Turkey Sandwich is a menu item comprised of bread AND turkey. SFA would need a Standardized Recipe.)

4) Processed Product Documentation
if schools choose to menu processed products, must provide documentation that demonstrates how the processed product contributes to the meal pattern.
Child Nutrition Labels (CN Label)
CN Labels are only for food products that contribute to the meat/meat alternate component. The label will provide the crediting of the creditable meat/meat alternate component in addition to all other creditable components within the food product. Note: CN labels will ONLY be on food products that contain a meat/meat alternate.

Manufacturer’s Product Formulation Statement (PFS)
When purchasing a processed product without a CN label, a program operator should have a completed and signed PFS. Manufacturers may use the form provided by USDA or on their own letterhead.
Note: If the food product contains more than one component, multiple PFS statements may be used. Example Chicken Nuggets will need a PFS for the Meat/Meat Alternate (chicken) and a PFS for Grains (breading).

NSLP USDA Fact Sheets
USDA Foods Fact Sheets are available for foods purchased through USDA Foods Distribution. Component contributions listed on the Fact Sheets may be used without any additional CN Label and/or Product Formulation Statement.
