We are currently receiving large volumes of applications via email and your email may bounce back on you saying that it is undeliverable. IF this happens, it is likely due to a large file size. To prevent the email from bouncing back, please only send the following items:
- 3 Page application
- Student’s birth certificate
- IEP/MET Cover Sheet (must include the student’s primary eligibility determination or special education category, date of the meeting, and student demographics)
Applications will be accepted January 1 - April 1 (3pm).
Please submit your child’s current public school district IEP or MET or 504 Plan along with your application. Funding is based on the primary eligibility of your child’s IEP or MET. No private evaluations will be accepted.
If your child’s IEP, MET, or 504 Plan is expiring on or before July 15, 2015, you must request a re-evaluation from your school district. The new IEP, MET, or 504 Plan must be submitted on or before April 1 by 3pm.
Please keep in mind that many districts might not be able to accommodate you with a re-evaluation immediately, therefore we recommend that you request your child’s re-evaluation as soon as possible.
