Home Welcome To Assessments AZELLA Test Security and Ethics Incident Report FY2027 AZELLA Test Security and Ethics Incident Report FY2027 Introduction AZELLA Test Security and Ethics Incident Report - 2026-2027 This form is used to report test security and ethics violations of Arizona's English Language Learner Assessment (AZELLA) occurring before, during, or after test administration. All incidents must be thoroughly investigated by the district and school prior to submitting this form. When the test security and ethics incidents involve the AZELLA, this secure report must be submitted to the Arizona Department of Education's Assessments AZELLA Team within 48 hours of the incident. ADE will respond within 3 full business days of the submission. If this incident involves students testing, the students may not resume testing until ADE provides the DTC with a response. Large-scale assessments like the AZELLA are developed, administered, and scored to meet comprehensive standards of validity, reliability, and fairness for a wide range of learners. In assessment, reliability is the characteristic of an assessment that allows its results to be repeatable. A reliable assessment will produce stable and consistent results. We can ensure these standards are maintained by closely adhering to security policies throughout test administration and conducting the assessment in a fair and ethical manner. The Arizona Department of Education, or ADE, is committed to taking every step required to assure the security and confidentiality of all state test materials, including the AZELLA. All AZELLA staff whether they are inventorying, distributing, or administering the tests, or just have incidental access to AZELLA materials must abide by these security standards. Failure to do so could negatively impact the validity of the AZELLA and the test results. Not abiding by the test security standards could also lead to allegations of test impropriety. Example: 12345678, Gr 7; 23456789, Gr 7 Date Incident Occurred AZELLA Test Security and Ethics Incidents Disclosing of student demographic information and/or student IEP or 504 Plan information with any unauthorized person(s) Allowing student(s) to leave or enter the testing room with secure testing materials including used scratch paper and testing tickets Using a cell phone during testing other than to report an emergency Reading and/or signing any test content to students except when scripted directions included in the ASL Test Administration Directions explicitly indicate to do so Rewording or clarifying questions or using inflections and/or gestures to help students choose the correct answer Interpreting test item(s) or passage(s) for student(s) Reviewing, discussing, and assisting a student with his or her answers, instructing and/or suggesting to a student to rethink his or her answers Instructing students to keep their test unit open after completing the unit Explicitly and/or implicitly encouraging students to engage in dishonest testing behavior Allowing students access to unauthorized resources to find answers, including having materials on walls or desks, or to additional resources such as books and worksheets while testing Sharing and/or exchanging User IDs and passwords with someone else for access to ADEConnect, TIDE, ClearCourse LMS, and/or Cambium's Test Delivery System (TDS) Providing access for an unauthorized user to TIDE; providing access to TIDE and testing materials to those who have not completed all training modules and test security requirements Logging into Cambium's TDS as a student for a regular online AZELLA test Allowing unauthorized personnel, including parent volunteers, to be present during testing and assisting with the proctoring and administration of the AZELLA Allowing school or district staff to administer and/or assist with proctoring and administration of the test their child is taking Using students to supervise other students taking a test Using unauthorized electronic devices in the testing room, unsupported testing devices, or an unsupported or beta version of an Operating System Using remote learning software to monitor students' computers and testing devices and view the actual screens of the students who are testing Copying (in written form or electronically) any part of the test, contents of the computer-based test, and test materials including but not limited to scoring guides, answer documents, and Test Administration Directions (TAD) Not returning test materials to the STC or DTC at the end of each daily test session Not storing test materials in a locked and secure location while not in use Losing secure test materials including but not limited to student testing tickets, test books, TADs, and student testing lists with student identifiable information Reviewing and analyzing secure test material Disclosing and/or discussing test content and/or items or performance tasks Erasing or changing student responses or altering any marks on a scorable test booklet or answer document, and student responses associated with an online test Developing scoring keys and/or preparing answer documents Using secure testing materials for instruction before, during, or after test administration including but not limited to instructing students to practice with operational test items Translating any of the AZELLA tests and test administration directions into other languages Taking pictures of paper test materials and testing devices that show the AZELLA tests Using Chromebooks and other capable devices converted to the tablet mode Other Identify the test security and ethics violation(s) involved in this incident.
