Application for Substantive Appeal of 2014 A-F Letter Grade

Please use this form to submit your Substantive Appeal of 2014 A-F Letter Grade.

 

You will need to complete the form, save the form, and then upload the form as an attachment to an email addressed to Achieve@azed.gov with 2014 A-F Substantive Appeal in the subject line.  Please include any supporting evidence you wish to submit with your application as attachments to the same email (required for expedited appeals) or present supporting evidence in-person at the non-expedited appeals hearing which will be scheduled by ADE.

 

EXPEDITED Submission Information

Submission Deadline: July 3, 2014
— PROCESS: Committee evaluates evidence using standardized rubric submitted via online or emailed form only. Schools will be notified by ADE of the committee’s decision.
Notification: August 1, 2014

NON-Expedited Submission Information

Submission deadline: August 1, 2014 by 12pm (noon)
— PROCESS: Schools should email the standard application form to Achieve@azed.gov; the submission may complete a brief narrative describing the reason for the appeal or choose to bring all appeal materials for the in-person hearing. The Committee evaluates evidence using a standardized rubric submitted via online form and school/LEA may appear before committee at a time scheduled by ADE to occur in August. Schools will be notified by ADE of the committee’s decision.
