Audit Evaluation Forms

The Arizona Department of Education (ADE) is required by the federal government to determine which sub-grantees of federal funds are subject to federal audit requirements. ADE requires all entities that receive federal funds passed-through ADE to complete an annual Audit Evaluation form. The completed form must be returned to the ADE Audit Unit no later than December 1, 2008. It is imperative that ALL federal funds expended by your agency be listed, regardless of whether those funds are received through ADE or another source

The Audit Evaluation must be completed by all entities regardless of amount of federal money expended. Those entities that expend $500,000 or more in federal funds during the previous fiscal year must undergo an OMB A-133 Single Audit. If no federal funding was received during the fiscal year in question, entities are still required to complete the evaluation and indicate "No Federal funds expended during previous fiscal year." 1) Memorandum and Audit Evaluation Form in PDF : or Word : The memorandum gives a brief explanation of requirements, Page One of the evaluation requests general information from entities such as name, address, and type of organization (e.g., nonprofit, local government, etc.). If a given entity received no federal funds during the fiscal year in question, that entity should check the appropriate box in item 9, and can then skip filling out Page Two of the Audit Evaluation, and send only Page One to ADE. If the entity received any federal funds, Page Two must also be filled out and accompany Page One of the Audit Evaluation.

Page Two of the Audit Evaluation requires entities to list (by line item) all sources and amounts of Federal funding expended during the previous fiscal year.

2) All federal funds are linked to a specific Catalog of Federal Domestic Assistance number (CFDA). The CDFA number is required to be included on the Evaluation form. The CFDA can be obtained using the following methods:

  1. ADE Grants Management system (top right hand corner of Project Summary)

  2. Internet search (Program Name CFDA)

  3. If have received funds in previous year, check Single Audit (Schedule of Federal Expenditures)

  4. www.CFDA.gov or Click here to download the current CFDA list.
Please note any funding for food programs is not processed through the ADE Grants Management system, instead use one of the options listed above.

Once completed and signed by an authorized entity representative, please return to ADE
by December 1st, 2008 :
